Returning Students:

Re-Enrollment Process for 2024-25

We are thrilled that you will be rejoining us in the 2024-25 school year!  Please complete all steps below:

Failure to complete your forms or pay Deposit/ Supply Fees by the due dates could result in a loss of placement at SMCS. 


SMCS uses Enrollease to help streamline student paperwork. Go to this link to register/ sign in. Form emails will be sent directly from Enrollease. Once registered, you will be able to complete the all necessary forms, as they become available, from your account.

Step 1- Intent to Return Forms:

Complete this form on Enrollease between March 4th and March 14th, 2024

This form indicates if you intend to return to SMCS for 2024-25, and informs how many spaces we give away in the lottery. All Intent-to-Return Forms are due by 3:00 pm, MST, on March 14th. Failure to submit this form by March 14th, 2024 will result in loss of placement at SMCS. We require an Intent-to-Return form for each student.

Enrollease will send you an invitation to complete on March 4th, and will expire by the end of day on March 14th.


Step 2- (PRESCHOOL) Contract, Program Agreement Forms & Deposit*:

Due by April 1, 2024, with $250 Deposit to reserve your space

Enrollease will send you an email with the reminder and invitation to complete on March 25th, and will expire by the end of day on April 1st.

*Deposits are non-refundable. Deposits become the yearly supply fee for the preschool students.

STEP 2- (K-8th) Supply Fee:

DUE April 29, 2024

The amount owed for each K-8th student for the 2024-25 school year is $150. We ask for this fee in lieu of asking you to buy supplies.

*If this presents a hardship for your family, please contact the HOS, Courtney Landry, at courtneylandry@salidamcs.org, or business manager, Hanley Harvey, at hanley_harvey@salidamcs.org. Supply fees are non-refundable. (This fee is waived for families qualifying for Free or Reduced Lunches)


Step 3- Re-Enrollment Forms**:

Complete this form between April 22nd and April 29th, 2024

This form is needed every year. As a school of choice, you must indicate that you are choosing us, by re-enrolling your student(s). Failure to submit this form by April 29th, 2024 will result in loss of placement at SMCS.

Enrollease will send an invitation to complete on April 22nd, and it will expire at the end of day on April 29th.

Step 4: Health Appraisal & Immunization Forms

Due by August 1st, 2024

Health Appraisal, Immunization Forms, & Copy of Insurance Card Due by August 1st, 2024. Please download the Health Appraisal Form (also available on Enrollease) and take it to the doctor. (Please note: that all Preschool-aged children are required to have a Health Appraisal Form- completed by a parent AND a physician). Also, you will need to submit a copy of your child’s Immunization Records or an Exemption Form and a copy of their Insurance Card.

You will not be permitted to attend school until these forms are received. If you need to schedule your appointment later, due to insurance timing, please notify the office of your appointment date.
